It looks as if there is something in the mouth area to prevent direct skin contact with the mask. Is this the case?

Around the mouth this has something like a mouth guard. This removes the discomfort of having to try and talk with the muffling from fabric. The fabric rests on your chin and face around your mouth area.

I am a female wondering what size I would order in the men's balaclava. I am 5'4" and 108#. Not sure if I should order xs or sm.

This brand does not provide a size chart. Unless you have a really small face I would say go with the small.
